Routine appointments are for non-urgent new health issues, review of existing health problems and long term condition management.

We do realise that not all problems are ongoing and sometines you do not expect to be poorly and may need to see a health professional quickly. 

If you have booked a routine appointment and your condition deteriorates, please telephone the surgery to book an emergency appointment. When the surgery is closed please telephone 111 or for an medical emergencies please call 999.

S: Seems different to normal. No matter how small the change think: Stop and Watch

T: Talks or communicates less

O: Overall needs more help

P: Pain (new or worsening). Participating less in activities

A: Ate less. Lack of appetite

N: No bowel movement in 3 days or diarrhoea

D: Drank Less. Monitor using a hydration chart

W: Weight Change. Losing weight without trying

A: Agitated or more nervous then usual

T: Tired, weak, confused or drowsy

C: Change in skin colour or condition

H: Help with walking, transferring or going to the toilet more than usual.
